Listed only as male Wells on birth record with parents Richard Wells (54, IN) and Mary Charlotte Cue (43, IL).
Parents also listed as Richard Wells and Mary Cue on death records.
Marriage record to wife Clara in 1913, who is referenced along with his mother on WWI Draft registration, where he is described as a teamster of medium height, grey eyes and light brown hair, living in Chicago.
But in 1920 census with his widowed mother, he is listed as single (divorced) and a coal driver.
In 1930 there is a census listing in Chicago on Pierce St for Sam Wells (40, IL) as a lodger with many others, working as a teamster in the cartage business whose parents were born in IL and CO.
His 1939 death in the Illinois records lists his burial in Proviso, Cook County, but that cemetery has only unmarked graves, with all others having been moved.

Daughter of Christopher (1839) and Anna C. (1849) Jacobs who immigrated from Germany in 1865/66 and were married shortly thereafter.
After her short marriage to Samuel Wells, she was quickly remarried to George Boyd Hemphill (1884-1964) on 14 Apr 1917 in Chicago.

Mrs. Clara Jacobs Hemphill
Services for Mrs. Clara Jacobs Hemphill, of Long Beach, Cal., will be held today in All Saints Episcopal church, Long Beach. Mrs. Hemphill was the widow of George B. Hemphill, a retired employe of The Tribune's traffic depart ment who died in 1964. Mrs. Hemphill died Sunday in Long Beach. She leaves two sons, John J. and George T.; and four grandchildren.
