He is listed as Charles W. Wells on his marriage record in 1928.
Living in Fredericktown in 1936, when his mother died.
His WWII Draft registration provides his middle name, wife Jessie and lists his birth as 22 Nov 1883.
SSDI lists his date of death, and Ohio Death Index provides the location as well noting that he was married (even though it lists his birth as abt 1885). It says he died in May 1959, and there are two death records for Charles M. Wells in May 1959, neither of which list an age, date of birth, or obituary:

- Charles M. Wells, died 26 May 1959 in Knox Co, OH

MRS. JESSIE WELLS
Mrs. Jessie Wells, 81, the Fairview Rest Home, died at 7:55 a.m. today in the extended care unit of Hocking Valley Community Hospital.
She was born May 2, 1894, in Monroe County, daughter of the late William and Rosetta Breckhammer Wheeler.
There are no known survivors.
Funeral services will be held at 2 p.m. Tuesday at Roberts Funeral Home.
Friends may call at the funeral home from 9 a.m. to 2 p.m. Tuesday.
