Other records indicate her birth in 1889, but her death certificate lists it as 1890.
She was married to Terry Smith (b. 27 Aug 1882 KY) on 15 Jan 1916 in Marion Co. IN (Bk 84, pg 232) with whom she had daughter Pearl 5 months earlier. They were separated by 1920, when she (29, IN) is married but listed again with her parents in Indianapolis with daughter Lucile (5, IN). William Baldwin (41, OH) a widowed boarder was living with them at the time, along with her cousins Dorothy and Ray Wells - children of Richard and Susan.
She later married William Baldwin and her mother came to live with them in Indianapolis following the death of her father. So, the family listed in the 1930 Indianapolis census is William Baldwin (52, KS), wife Fairy (39, IN), step-daughter Pearl (Lucile) Smith (15, IN) and Fairy’s widowed mother Mary (72, IN). It says the Wm was a carpenter and first married at age 23, Fairy first married at 22 (1911?), and Mary first married at 25.
After William’s death, Fairy married a 3rd time in 1935 to Joseph Thompson Proctor (b. 10 Jan 1884 Palmyra, Montgomery Co, TN) and in 1940 they were listed in Mars Hill, where they had been since at least 1935. He was a stockman in a box factory, and living with them was her mother Mary at age 82. It lists Fairy in the supplemental data as having been first married at age 17 (1906?), having born 2 children and completed her sophomore year of High School.
Her husband Joseph registered for the WWII Draft in 1942 and listed Fairy as his next of kin, and his employer as American Valve and Enameling Corp. They lived outside Indianapolis in Decatur Twp.
Her Texas death certificate as Fairy Maude Proctor lists her parents, and says she was married at the time of death. They had lived in San Antonio 5 months.

Listed as an Ironworker when he registered for the WWI Draft in 1918. He and Fairy were living at 2467 Columbus Ave in Indianapolis at the time.
There is a Terry H. Smith listed in the 1920 Indianapolis City Directory, as a manager for Sam’l Feldman. He and Fairy were separated and she was subsequently remarried a couple more times.

He was married to Clara Hardee on 28 Nov 1901 in Indianapolis. He is listed as Herbert W. Baldwin in the 1910 census, working as a carpenter (32, OH) married 8 yrs with wife Clara (25, IN) and children Elmer A. (7), Laoma (5), and Clarence (3); living on Walnut St. They were still there when he registered for the WWI Draft in 1918. And Clara is listed on Walnet St as head of household with the children in 1920.
He is listed as a widowed carpenter (41, OH) working as a contractor and living with the Wells family as a boarder in 1920 on Columbia Ave in Indianapolis.
In 1930 he is listed on Pershing Ave in Indianapolis as a carpenter (52, KS) with his wife Fairy, step-daughter Pearl Smith and mother-in-law Mary Wells. It says he was first married at age 23.

WILLIAM H. BALDWIN.
William Herbert Baldwin, 55 years old, 1405 North Belmont avenue, died Wednesday in his home after a brief illness. Mr. Baldwin was born in Marietta, 0., Dec. 30, 1877, and spent his early life in Scottsburg, coming to Indianapolis when he was 20 years old. Survivors are the widow, Mrs. Fairy Baldwin; three children, Elmer, Leona and Clarence Baldwin; a grandson, Kenneth Baldwin, all of this city; three sisters, Mrs. Lillie Thompson, Mrs. Myrtle Kulmer and Mrs. Lulu Kulmer of Louisville, Ky., and three brothers, Earl Baldwin of Port Arthur, Tex., Edward Baldwin of Terre Haute and Warner Baldwin of Buffalo, N. Y. Funeral services will be held at 2 o'clock tomorrow afternoon in the home. Burial will be in Floral Park cemetery.
